By completing this application, I am applying for membership in OSHP. I agree to abide by the bylaws, support the mission of the organization, attend meetings when possible, pay the appropriate dues, and follow all established policies and guidelines. Membership dues are based on each individual’s anniversary year.

OSHP dues are not deductible as a charitable contribution for federal income tax purposes but may be partially deductible as a business expense. OSHP estimates that 65% of your dues are not deductible because of OSHP’s lobbying activities on behalf of its members.

Pharmacist Members shall be licensed pharmacists with a board of pharmacy, or retired pharmacists who actively support the objectives of OSHP. The privileges of Pharmacist Members shall include, but are not limited to, voting on all matters submitted to them and holding elective or appointed positions after meeting all other qualifications.

Supporting Members may be individuals, other than health-system pharmacists, who by their work in the health services, the teaching of prospective health-system pharmacists or pharmacy technicians, or otherwise contributing to health-system pharmacy, make themselves eligible for membership.

A New Practitioner Pharmacist Member is any pharmacist in their first seven (7) years of practice.    

A Retired Pharmacist Member is available to applicants who are 62 years of age or older.

Technician Members shall be licensed, registered, and/or certified pharmacy technicians with a board of pharmacy.

Student Members must be enrolled in graduate or undergraduate programs in accredited colleges or schools of pharmacy.
